- Under the direct supervision of the Information and Referral Coordinator, this position completes in-person assessments with nursing facility residents wishing to return to a community based setting, conducts in-person or telephone assessments with individuals referred for community living screenings (CLS), and provides in-person and telephone screenings for Ohio Benefits Long Term Services and Support (OBLTSS) contacts. The Community Living Navigator also assists customers by telephone, electronic means and in person to identify and locate community resources and programs, using a variety of resources (electronic, print and others).
